Shea-Porter Announces $200,000 for NH Department of Health and Human Services

Washington, D.C. – Congresswoman Carol Shea-Porter today announced that the New Hampshire Department of Health and Human Services will receive $200,000 to provide self-management programs to older adults with chronic diseases, build statewide delivery systems, and develop the workforce that delivers these programs.  This grant is provided by the “Communities Putting Prevention to Work Chronic Disease Self-Management Program” of the U.S. Department of Health and Human Services.  Funding is made possible by passage of the American Recovery and Reinvestment Act.
